2/19/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C met with Mykei today. Mykei appeared to be in a positive mood evident by his facial expressions such as a smile on the face and his overall disposition. QMHP-C informed Mykei that this session was going to include his mother. Mykei did not appear to be so happy once QMHP-C informed him of that information; however, QMHP-C explained to him that a family session has to be conducted.. 2. Behavior: Mykei presented cooperative and normal behaviors as he did not need any redirection from QMHP-C as well as him answering open -ended questions and being praised by QMHP-C. At times within the session, Mykei needed assistance with being able to verbalize his feelings of frustarion. 1.Intervention: QMHP-C assessed and monitored Mykei’s mood and behavior. QMHP-C probed in order to process and explore Mykei’s lack of social skills. QMHP-C educated and reviewed effective communication skills with Mykei to utilize with family members and others (socially). QMHP inquired whether Mykei was exposed to anyone diagnosed with Coronavirus and discussed safety tips to protect from contracting COVID-19. QMHP-C provided family with the following interventions such as wash your hands regularly for 20 seconds, with soap and water or alcohol-based hand rub, Cover your nose and mouth with a disposable tissue or flexed elbow when you cough or sneeze, Avoid close contact (1 meter or 3 feet) with people who are unwell, Stay home and self-isolate from others in the household if you feel unwell. QMHP-C expressed to family “Don't touch your eyes, nose, or mouth if your hands are not clean”. QMHP-C educated and reviewed appropriate coping strategies with Mykei to utilize within social settings to prevent Mykei from withdrawing and distancing himself. QMHP-C modeled appropriate skills and strategies through role play and role reversal. 2. Intervention: QMHP-C encouraged Mykei to increase his social interactions with others. QMHP-C discussed the importance of positive relationships with his family and peers. QMHP-C encouraged Mykei to articulate and express his feelings without exhibiting inappropriate and disrespectful behaviors. QMHP-C prompted Mykei to apologize to Ms. Shabazz and family for his behavior today. 1. Response: In todays’s session Mykei reported that he has been doing what he is supposed to be doing but it is his siblings that he getting him in trouble causing him to get upset and for him to leave the home and go to the playground when he wants to. Mykei stated that he will not utilize aggression towards his mother nor family members if they just stay out the way. 2. Response: Mykei reports that he is going to do what he suppose to do when it comes to listening to his mother and his grandmother. He reports that he is not trying to get in trouble with is mother because she will take his phone away from him and that is something that he does not want to happen.
